Ontario LTB N Forms for Tenants

Tenant Notice Forms
N9 – Tenant’s Notice to End the Tenancy
Use this form when a tenant wants to give notice to move out. Tribunals Ontario’s tenant brochure says that if a tenant wants to end the tenancy and move out, they must either give written notice or make an agreement with the landlord to end the tenancy, and to give notice they must use Form N9. The official N9 form says that for most tenancies, including monthly tenancies, the termination date must be at least 60 days after the tenant gives notice, and it must usually be the last day of the rental period.
N11 – Agreement to End the Tenancy
Use this form when both the landlord and tenant agree to end the tenancy together. This is not a one-sided notice. It is a mutual agreement signed by both parties. The official N11 form also says a landlord generally cannot require a tenant to sign it as a condition of renting a unit, subject to limited exceptions listed in the form.
N15 – Tenant’s Notice to End my Tenancy Because of Fear of Sexual or Domestic Violence and Abuse
Use this form in specific legally protected situations where the tenant, or a child living with the tenant, is experiencing sexual or domestic violence and abuse and believes harm may occur if they remain in the unit. The official N15 form explains that if there is a joint tenancy, some or all tenants can sign the notice to end their own interest in the tenancy.
